Couple and Relationship Work ($150 USD per 60-minutes)
Family Therapy ($150 USD per 60-minutes)
Payment is received by credit card.
INSURANCE & PAYMENT
I am a preferred provider with Aetna, Blue Cross/Blue Shield, and Premera. I am unable to bill Medicare and Medicaid.
If you are a subscriber to other insurance companies, I am pleased to provide detailed receipts and claim forms that meet the requirements of insurance companies for the purpose of reimbursement. Timely submission of receipts and claims generally will provide reimbursement to the client. To better understand the process and how to go about obtaining reimbursement, the following questions can be helpful when contacting your individual insurance company:
Do I have mental health benefits?
Do I have a co-pay? If so, how much is it?
What is my deductible and has it been met?
How many sessions per calendar year does my plan cover?
Does my plan cover an out of network provider?
Is approval required from my primary care physician?
How often will you need to submit your claims?
How long does it take to receive reimbursement?
